How to make a claim


Claims paid out within 48 hours of receiving all the required documents

Required documents:

  • Completed and signed Claim Form
  • Death Certificate of member
  • Certified copy of the ID of person making the claim and/or the beneficiary
  • Notice of Death (Bi1662)
  • Police report (in the event that death occurred as a result of unnatural causes)
  • Certified copy of the Antenatal Chart (in the event of a still birth)
